Most patients who find me have already tried PT. It didn't work. Here's why this is different.
At Physica Medica, every session is one-on-one with me, Dr. Birikov — not a rotating aide, not a shared gym floor, not 20 minutes and out. I treat the root cause of your pain, not just the symptom that brought you in. If you're done managing pain and ready to fix it, this is where that starts.

One-on-One Physical Therapy with Dr. Birikov
This is the foundation everything else is built on. A full session, start to finish, with me. No handoffs. No assistants finishing what I started. I assess, I treat, and I adjust the plan based on what I find — every single visit. That's not how most clinics operate. It's how this one does.
See how it worksManual Therapy & Hands-On Techniques
Deep, targeted hands-on work to release tight fascia, break up muscle knots, and restore movement that pain has been blocking. This includes deep tissue massage, a metal scraping tool applied directly to restricted tissue (IASTM), and suction cup therapy (cupping) to pull blood flow into areas that aren't healing on their own. These aren't spa treatments. They're clinical tools with a specific mechanical purpose.
Posture Correction & Spinal Alignment
Rounded shoulders, forward head, a hunched back — these aren't cosmetic problems. They're structural patterns that load your spine wrong and keep pain coming back no matter how much you stretch or strengthen. I find the pattern causing yours and correct it at the source.
Specialty & Imported Techniques
I traveled internationally to train in methods most American clinics have never offered. Pancafit — a global stretching and postural correction system — is one of them. Physica Medica is the only Pancafit studio in the United States. When conventional approaches haven't moved the needle on your pain, these are often what finally do.
Fitness, Strength & Wellness
Rehab doesn't end when the pain stops. I rebuild the body so it holds — with clinical fitness programming designed around your specific injury history, your sport, and what you're trying to get back to. Getting out of pain is step one. Staying out of it is the actual goal.
Bring me the pain that hasn't responded
Most patients who come to Physica Medica have already seen someone else. A general PT. A chiropractor. Maybe a surgeon who said to try conservative treatment first. The conditions I treat most — chronic back pain, herniated discs, sciatica, rotator cuff injuries, IT band syndrome, plantar fasciitis, tension headaches, nerve pain, ACL recovery — are exactly the ones that get mismanaged when a clinic is moving too fast to find the actual cause. I'm not faster. I'm more thorough.

What conditions does physical therapy treat?
Physical therapy addresses musculoskeletal pain, nerve pain, post-surgical recovery, postural dysfunction, and movement limitations caused by injury or chronic tension. At Physica Medica specifically, I treat back pain, sciatica, herniated discs, rotator cuff tears, ACL injuries, IT band syndrome, plantar fasciitis, tension headaches, and more. If you're not sure whether your condition fits, call and tell me what's going on. I'll tell you honestly whether I can help.
How do I know if physical therapy is right for my condition?
You don't have to figure that out before calling. On your first visit, I assess your body, your history, and what's actually driving your pain — then I tell you what I think will work and why. If physical therapy isn't the right answer for what you have, I'll tell you that too. I'd rather give you an honest assessment than take your money for a plan that won't produce results.
Do you treat chronic pain, or only acute injuries?
Chronic pain is the core of what I do. Acute injuries are straightforward — the body knows how to heal a fresh sprain. Chronic pain is harder because the original injury is often long gone and what remains is a compensatory pattern, restricted tissue, or a structural problem nobody has addressed directly. That's where one-on-one care, time, and a root-cause approach make the difference. There are no assistants, no aides, and no shared gym floor at Physica Medica. If you've had chronic pain for months or years and nothing has worked, that's not a reason to give up — it's a reason to try something structurally different.
