Insurance-based PT has a structural problem: the insurer dictates the care. It sets how many minutes you get, caps how many sessions you're approved for, and limits which techniques are reimbursable. That's why so many clinics run a shared gym floor with aides. It's the only way to make the economics work under those rules.
