Specialty Techniques in Baltimore

Methods most Baltimore clinics have never heard of. One doctor who trained to bring them here.

Standard physical therapy works for straightforward cases. But if you've already been through PT and you're still in pain, the issue probably isn't your effort. It's that the tools were wrong for the problem. I spent years training in Italian and Eastern-origin methods specifically because they reach what conventional PT misses — chronic nerve pain, postural dysfunction, breathing mechanics, deep fascial restriction. These aren't trendy add-ons. They're the reason patients who had given up start getting real results.

The Foundation

Everything starts with one-on-one physical therapy

Before any specialty technique gets applied, I need to understand your body. Every session at Physica Medica is one-on-one, start to finish, with me. No assistants, no aides, no rotating staff. That structure isn't a selling point — it's what makes accurate diagnosis and consistent treatment possible. It's also what most clinics can't offer.

01

Manual Therapy and Hands-On Techniques

Deep tissue work, metal scraping tools (IASTM), cupping, and dry needling — techniques that go after tight fascia, muscle knots, and restricted tissue directly. These aren't spa services. They're clinical tools with documented mechanisms, and I use them when the assessment says they're the right fit.

02

Posture Correction and Spinal Alignment

Rounded shoulders, forward head posture, a hunched back — these patterns don't fix themselves with stretching. I use Pancafit, the only postural alignment system of its kind in the United States, to correct the root structural cause. If your pain keeps coming back after treatment, posture is often why.

03

Specialty and Imported Techniques

I trained abroad to learn methods that aren't taught in standard American PT programs. Pancafit for postural realignment. Neural tension release for nerve pain that runs down the arm or leg. Diaphragmatic rehabilitation for patients with breathing dysfunction — including singers and performers. These techniques exist because some conditions need a different approach entirely.

04

Fitness, Strength, and Return to Sport

Pain going away is not the finish line. I rebuild the body so it holds — with clinical Pilates, strength programming, and sport-specific work. Athletes get back to training. Non-athletes get back to living without bracing for the next flare-up.

Conditions Treated

Bring me the pain that hasn't responded

Sciatica, herniated disc, rotator cuff injuries, IT band syndrome, plantar fasciitis, tension headaches, nerve pain, ACL recovery, chronic back pain. These are the conditions patients carry in after months — sometimes years — of treatments that didn't hold. Every technique I use exists to address a specific mechanism behind a specific problem. If you've been told your case is complicated, that's exactly the kind of case I take.

Every Session. Same Doctor. No Exceptions.

You'll always know who you're seeing

Whichever service is right for you, I'm the one delivering it. Every session, start to finish. That's not how most clinics operate — and it's exactly why patients who've been through the system before notice the difference immediately.

Physica Medica is a cash-pay practice. Pricing is discussed on your first call — no surprises. See payment and insurance details →

Questions before you pick a service

Do I need to know which technique I need before I call? No. That's my job. On your first visit I assess your movement, your history, and what's actually driving the pain — then I tell you exactly what I'm going to use and why. You don't need to arrive with a treatment plan. You need to show up.

Are these methods actually evidence-based, or are they alternative medicine? Each technique I use has a documented clinical mechanism. Pancafit was developed in Italy by Costantino Pirri and has been used in European rehabilitation for decades. Neural tension release addresses the mechanical behavior of nerves under load — it's anatomy, not theory. Diaphragmatic rehabilitation is grounded in respiratory physiology. I don't use anything I can't explain and defend. The 300 five-star reviews from patients who had already tried other approaches are the outcome data.

Will I see Dr. Birikov at every session? Every one. There are no assistants, no aides, and no shared gym floor at Physica Medica. When you book with me, you get me — not a technician who runs you through a protocol while the doctor checks in for five minutes. That consistency is what makes it possible to actually track progress and adjust the approach when something needs to change.
