17 ways to fix pain. One doctor who delivers all of them.
Most Baltimore PT clinics hand you a narrow menu and a rotating cast of therapists. I do something different. Whatever technique your body needs, whether that's hands-on manual therapy, dry needling, or imported methods you can't find anywhere else in the country, you get it one-on-one with me, Dr. Maksim Birikov. Same doctor, every session.
Treating chronic and complex pain, one patient at a time.
Five-star Google reviews from real Baltimore patients.
The only Pancafit studio in the United States, right here in Fells Point.
One-on-One Physical Therapy
This is the foundation everything else is built on: a full session, start to finish, with me. No aides, no shared gym floor, no protocol sheet. If you've tried standard PT and it didn't get results, this is what that looks like done differently.

Manual Therapy & Hands-On Techniques
Deep, targeted hands-on work to release tight fascia, break up muscle knots, and calm the trigger points driving your pain. I start most patients here, led by dry needling, the technique I'm certified for in Maryland.
Posture Correction & Spinal Alignment
Rounded shoulders, forward head, a hunched back, a curved spine. These aren't cosmetic. They load your joints wrong and drive chronic pain. I correct the alignment at the root, led by my scoliosis work.
Specialty & Imported Techniques
I traveled to find methods most American clinics have never heard of, led by Pancafit, of which this is the only studio in the United States. These aren't gimmicks. They're tools that work when standard PT stalls.
Fitness, Strength & Wellness
Rehab doesn't end when the pain stops. I rebuild the body so it holds, with clinical-grade strength and conditioning guided by a physical therapist, not a gym trainer. Led by one-on-one personal training.
Bring me the pain that hasn't responded
Every technique above exists to solve a problem. These are the ones patients come to me with most.
Before you pick a service
You don't have to. That's my job. On your first visit I assess your body, your history, and what's actually driving the pain. Then I choose the techniques that fit. Most patients get a combination, not a single service. You're not buying a menu item; you're getting a plan I build around you.
Yes, and you usually will. Because I run the full session myself with no time pressure from an insurance clock, I can combine dry needling, manual therapy, cupping, and movement work in one visit based on what I find that day. A protocol-driven clinic can't do that. I adjust in real time.
Every one. There are no assistants, no aides, and no shared gym floor at Physica Medica. Whether it's dry needling, Pancafit, or strength training, you work directly with me for the entire session. That's the whole point of the practice.
